Job Description
The Care Coordination Administrative Assistant will support our Community Care Coordinators by accurately transcribing and entering their notes into our internal computer software and systems. This position offers an opportunity to contribute to the efficient coordination of care for our clients and make a meaningful impact in their lives.
Responsibilities include, but not limited to:
· Assist Community Care Coordinators by transcribing and accurately entering their notes into our internal computer systems and software.
· Apply critical thinking skills to ensure the accurate capture of essential information from care coordination sessions.
· Review notes for completeness and clarity, seeking clarification from Community Care Coordinators as needed.
· Organize and maintain documentation in an orderly and accessible manner.
· Collaborate with Community Care Coordinators to follow up on any discrepancies or missing information.
· Provide administrative support to Community Care Coordinators as needed, including scheduling appointments and managing correspondence.
· Maintain confidentiality and adhere to privacy regulations when handling sensitive information.
· Additional Duties as assigned.
