Discharge Planner - Home Care jobs in Louisiana
Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Essential Functions
- Develop continuing after care plan, based on patient's needs, to promote long term health and relapse prevention.
- Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist.
- Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
- Act as a liaison between referral sources, patients and counselors.
- Ensure all patient discharge planning documentation is clear, concise and timely.
- Perform other functions and tasks as assigned.
- Associates degree in nursing or human services field required. Bachelor's degree in social work or human services field preferred.
- One or more years’ experience in a clinical setting preferred.
- One or more years’ experience in discharge planning preferred.
- If applicable, current licensure as a LPN, RN within the state where the facility provides services.
- CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.
- First aid may be required based on state or facility requirements.
