Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work.     Our education program is accredited through the American Association of Diabetes Educators (AADE). Patients managing diabetes don’t have to do it alone. We provide the necessary tools so patients can successfully self-manage the day-to-day rigors of living with diabetes. A certified diabetes educator will work with your patient to create a plan that provides ongoing support and tools for improved health. Our education services include Diabetes self-management training, Medical nutrition therapy, Insulin pump therapy and training, Diabetes and exercise strategies, Diabetes medication education, Weight management, Pregnancy and diabetes education and counseling, Continuous glucose monitoring therapy and training, and Blood glucose pattern management.

    Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. 

    Scope of Work

    Provides quality education, counsel, and support to clients and their significant others regarding diabetes self-care. Documents the education process, patient information and clinical recommendations according to department/program/regulatory guidelines, and measures outcomes to maximize continuity of care between settings. Participates in projects related to area of specialty and the development/revision of procedures/policies, educational materials/guidelines, educational programs. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community.

    Qualifications 

    • Bachelor's Degree Nursing (BSN) or related healthcare field Required
    • Master's Degree Preferred
    • 3 years of relevant experience recent, relevant clinical experience including prior teaching experience with adult clientele Required
    • MI Registered Nurse (RN) Required Or
    • Registered Dietitian (RD) CDR Commission on Dietetic Registration Required
    • AHA CPR Required within 90 Days
    • Certified Diabetes Educator (CDE) - NCBDE National Certification Board for Diabetes Educators Required within first 3 years
