The Registered Nurse Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to our clients in their homes. The RN Case Manager conducts day-to-day nursing activities, to include assessment and education, to our clients in a one-on-one setting in their home. The RN Case Manager for Saint Elizabeth Home Care is a part of a cohesive and collaborative interdisciplinary team with ample administrative support. Our RN Case Managers are equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.
