How to Enroll - A Three-step Process The student must complete an enrollment packet. Packets can be obtained at the FDA office - 1219 W. Main Cross, Suite 101, or from any Findlay High School counselor or principal. The packet can also be downloaded from our web site at http://fda.findlaycityschools.org The student and parent (if the student is under 18 years of age) need to call the FDA office (419-425-3598) and set up an appointment with Mrs. White or Mr. Grove. Attend orientation at the Findlay Digital Academy office at 1219 West Main Cross, Suite 101, Findlay. Orientation is a two-day process. On the first day the student and parent must attend a two-hour orientation. On the second day, only the student needs to attend another two-hour orientation session. Upon completion of the second day of orientation, the student is officially enrolled in Findlay Digital Academy. Student Assessment Teachers evaluate student work and give one of the following qualification levels: Level 3 (A): The student shows a deep understanding of the lesson.
