Care Alliance is a recognized NCQA Patient-Centered Medical Home and Federally funded health center dedicated to serving the needs of homeless, people in and around public housing, uninsured and underserved residents of Cuyahoga County. In 2018, Care Alliance served more than 14,900 patients, 90% of whom fall below the poverty line. Intensive outreach efforts and strong partnerships with other agencies allow Care Alliance to deliver primary medical care. What began in 1985 as Cleveland's "Health Care for the Homeless" has grown to encompass primary comprehensive medical care, dental care, behavioral health, and other supportive services in Cleveland. Through strategic partnerships with other safety net agencies, and the support of foundations, corporations, and residents of the community, CAHC continues to be a medical home for Cleveland residents. As a Federally Qualified Health Center, the organization is able to offer services to patients of all ages on a sliding fee scale that is based on one's ability to pay.
