What are the responsibilities and job description for the Medical Case Manager position at Unity Place of Monmouth County?
We are currently seeking a qualified Prescriber’s Assistant candidate for our Mental Health Partial Care/ Addictions Outpatient program. The hours for this position are Monday-Friday 8:30 AM - 4:30 PM.
The Prescriber’s Assistant position will have the following job responsibilities:
- Develop weekly schedule for Psychiatric Evaluations, Psychiatric Reviews, and Medication Management appointments
- Review all documents prepared by Psychiatrist/APN for accuracy
- Send copies of medication lists to residential healthcare facilities or group homes
- Manage injection schedule and bloodwork schedule
- Review that all clients are receiving medication education and fact sheets
- Monitoring of medication room and ensuring that inspection of medication storage by pharmacist is according to regulations
- Facilitation of psycho-educational groups as needed
- Case management
Skills/Qualifications:
- Ability to work with mental health, addictions, and co-occurring clientele
- Efficient oral and written communication and decision-making skills
- Strong time management and organizational skills
- Creativity and flexibility
- Team player, professional, friendly
- Basic computer literacy
Qualified candidates MUST HAVE a license, including LSW OR LAC
