One Vision is looking for a Full time - Outcome Coordinator for our Fort Dodge HCBS Team!

Our Outcome Coordinators coordinate and integrate a Life Plan for assigned individuals of One Vision (OV) , monitor the Life Plan and progress on goals / objectives and supports to ensure persons served needs are met .
Qualifications :
BA / BS degree in human service-related field or RN license with 1 year of verifiable experience in supporting individuals with disabilities is preferred.
OR High school education or GED with 4 years' of progressively responsible experience in supporting individuals with disabilities.
Depending on team assignment, must be certified medication aide or medication manager, if not currently held, must become certified with six months.
Job Skill Requirements :
- Moderate Computer skills needed for documentation.
- Ability responds appropriately to behavior, crisis, and emergencies to utilizing Mandt procedures.
- Valid driver's license with proof of insurance.
- Requires some physical activity bending, kneeling, and lifting or use of lifts.
- Offer of employment contingent upon background check and drug screen.
