Location: Lakeview Health Center & Community Living is a state-of-the-art campus in West Salem, WI.
Shift: 10 p.m. to 6:30 a.m., Friday & Saturday every other weekend. Additional hours are available! Staff also work a holiday rotation.
Wage depends on experience!
- Up to $43.50/hr and $46.50/hr on weekends with differential.
Provides care within the provisions of the Nurse Practice Act. Provides direct nursing care including administering medications and treatments. [1]
Supervises all nursing activities on assigned living area. [1]
Evaluates and determines resident needs and initiates and revises the resident care plan. Contributes to the multidisciplinary resident care planning. [2]Provides for implementation of all medical orders; assists physician, as needed; collaborates with the Nurse Managers to prepare information for physician's rounds. [1]
Makes clinical assessments of resident condition and behaviors. Determines appropriate action based on these assessments. Makes accurate and complete nursing notes of these observations and actions in the clinical records. [1] Documents accurately and completely all observations, interventions and procedures relating to resident care. [3]
Directly supervises the performance of nursing tasks assigned to Nurse Technicians. [1]
Determines and supervises work assignments to provide maximum utilization of Nursing Assistants and is responsible to see that assigned tasks are accomplished. [3]
Provides feedback to nursing assistants regarding job performance. [3]
Assists in the training and orientation of new staff. [1]
Ensures correct and appropriate diet for residents.{1}Documents all observations and procedures relating to resident care accurately and completely.[1]
Prepares daily shift reports and keeps supervisory nurse informed. [3]
Employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness. [3]

MDS ESSENTIAL JOB FUNCTIONS HILLVIEWResponsible to assist the MDS Coordinator to provide for the timely and accurate completion of the MDS (Minimum Data Set) for nursing home resident assessment and caring planning. Will also assist with the Restorative Nursing program.
Demonstrates strong clinical nursing skills in assessment, planning, implementation and evaluation of nursing interventions for facility residents. Responsible for the accurate and timely completion of the MDS, as required by OBRA/Federal regulations and PPS.
Coordinates and/or participates in care planning process, especially for residents with complex care needs.
Demonstrates knowledge and clinical competence in area of resident care standards.
Thorough understanding and working knowledge of the current MDS and RUGS systems.
Thorough knowledge of application of the nursing process, assessment, planning, implementing and evaluating, completing accurate assessments and MDS entries that will result in correct resident assignment to the proper RUGS IV to ensure the appropriate payment to the facility.
Ability to maximize RUGS scores through accurate MDS coding by capturing restorative participation, skilled nursing interventions and therapy.
Physical demands: Up to 75% of time is spent standing and talking. Walking, hearing, and using near vision is used about 50% of the time. About 25% of time is spent sitting, bending, twisting, reaching. Reads and writes. Activities done about 10% of time include stooping crouching, low to very high lifting - up to 60 lbs. to transfer residents and pushing residents in wheelchairs. Uses nursing related equipment.
