What will I be doing?
Providing individual and family individualized treatment plans
Counseling children and families in preparation for placement or discharge
Supervising children in foster homes; preparing and maintaining case records; coordinating services to minimized fragmentation of care, reduce barriers and link children with appropriate services to ensure comprehensive, continuous access to needed medical, social education and other services appropriate to the needs of the child and ensuring continuity of care for a child in treatment foster care.

Ability to facilitate relationships between birth parents and foster families.
Collaborates with family assessment and planning teams and other involved parties on reviews and coordination of services to children and their families.
Employee must Independently transport self in his/her own vehicle to provide services in the in-home/community-based service milieu, attend meetings; and if necessary, transport clients occasional. Employee must agree & accept this job duty requires his/her to maintain a current driver’s license, as safe driving records, current vehicle insurance and always maintain his/her vehicle in safe operating condition.
Qualifications:
- Bachelor's degree in related field
- Experience providing casework services to children and families
