The Pharmacist will work with a team of pharmacy professionals and other health care providers to perform a variety of routine and enhanced pharmacy services including preparing and dispensing drugs in accordance with standard procedures while ensuring optimal health outcomes. The ideal candidate must be able to balance efficiency with accuracy, work well both independently and with a team in a fast-paced culturally diverse environment as well as have flexible scheduling availability.

    Work Location: Duke Regional Hospital location at Lincoln Community Health Center – Durham, NC

    Working Hours

    • Day shift 20 hours/week
    • 9:00am - 6:00pm on Monday and Tuesday; 2:00pm – 6:00pm on Wednesday
    • Occasional rotating 11:00am - 8:00pm on Tuesday and 9:00am – 1:00pm on Thursday

    General Description Of The Job Class

    Provide pharmaceutical care services to the patients of the Duke University Health System and/or community partners/affiliates including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems; support business and operational needs; ensure compliance with Federal, State and local laws and regulations as well as proper storage and documentation. Maintain compliance with all accrediting bodies, third party vendors and patient assistance programs along with the required documentation. May also advise Providers on matters pertaining to drug usage and control; participate in the development and delivery of continuing education programs for Pharmacy, Patient Care Services, Medical Staff and students.

    Duties And Responsibilities Of This Level

    Review and monitor patient's medication therapy and orders written by Providers for safety and efficacy considering specific patient conditions and health status; advise patient care team of issues with, or possible enhancements to, patient's medication therapy and document such interventions.

    Conduct and evaluate medication histories, assess compliance and suggest modifications to achieve desired outcomes.

    Provide drug information and educate Providers, caregivers, patients and students regarding medications and their proper use and monitoring. Ensure patient /caregiver understanding of intended treatment and anticipated outcomes, by utilizing DUHS/ community partner/ affiliate translation resources when needed.

    Oversee and monitor all aspects of the medication process including safety; collect data and present reports and recommendations regarding trends and issues identified with the medication systems and safety if assigned.

    Oversee and monitor medication adherence initiatives; collect data, reconcile reimbursements and present reports and recommendations regarding trends and issues identified if assigned.

    Identify and implement IT opportunities to improve care, compliance and safety for patients in the various electronic systems used by the department.

    Prepare, label and dispense medications for patients in accordance with providers' prescriptions; ensure compliance with Federal, State and local laws and regulations pertaining to the dispensing and administration of drugs and narcotics and the maintenance of required records. Periodically review participation requirements and maintain compliance with all accrediting bodies, third party vendors and patient assistance programs along with the required documentation if assigned.

    Ensure that appropriate medications are available for patients and properly procured, prepared, stored and maintained.

    Oversee the process, required documentation and technical personnel who assist in procurement, preparation, storage, maintenance, distribution and dispensing of drugs and supplies.

    Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising providers on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses. Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.

    Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls and Joint Commission standards. As applicable, provide timely information to regulatory bodies, third party vendors and patient assistance programs along with the required documentation.

    Conduct and participate in research, if assigned, including, but not limited to, performing as the primary investigator or co-investigator for research programs which will impact on the delivery of quality care or examine the pharmacoeconomic impact of providing care to the patients in the Duke Health System and/or community partners/affiliates.

    Collate and analyze data collected by the department to prepare posters and journal articles for sharing knowledge at conferences and publications if assigned.

    Perform other related duties incidental to the work described herein.

    Minimum Qualifications

    Education

    • Doctor of Pharmacy Degree OR BS Pharmacy Degree

    Experience

    • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.
    • With Doctor of Pharmacy degree, no experience is required.

    Degrees, Licensure, and/or Certification

    • Licensed Pharmacist or eligible to practice pharmacy in North Carolina

    Knowledge, Skills, And Abilities

    • Ability to communicate clearly in oral and written form.
    • Excellent clinical skills; extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states.
    • Ability to work with information systems, clinical platforms, and automated and technical equipment; including Microsoft Word, Excel and Power Point.
    • Ability to establish and maintain collaborative relationships with various health care team members to deliver patient centered care while demonstrating the value of pharmacy services.
    • Ability to remain focused, accurate and efficient in pharmacy workflow while providing excellent customer service in an empathetic manner. Able to flex easily with changing workflow and department priorities.
    • Must be self-directed, seek personal and professional growth, demonstrate initiative, have excellent problem solving skills and be highly organized with attention to detail.
    • Demonstrates teamwork within the Pharmacy Department and participate in the group approach to patient care by providing support to peers and other practice areas as needed in addition to daily assignments.
    • Demonstrates reliability and accountability to Pharmacy Department, colleagues, patients and healthcare professionals.
