Job Summary The Social Services Director provides medically-related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident. They will assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility’s Social Service Department in accordance with current federal, state, and local standards, guidelines and regulations, and facility established policies and procedures. This position reports to the facility Administrator. Social Services Director will: • Appropriately transition the resident and family through the care process • Access cognitive capacity and mood assessments utilizing standard tools • Implement healthcare directives and ability to have discussions and knowledge of end of life care and hospice • Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician Our Social Services Director must have: • MUST have minimum 2-3 years experience in long term care and and skilled nursing care • Must have a Bachelors in social work • Must demonstrate strong conflict resolution • Therapeutic relationship skills • Insurance/Medicare proficienc
