Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. The Diabetes Coordinator will manage the SDPI (Special Diabetes Program for Indians) grant, provide supervision of SDPI staff and complete applications for additional funding opportunities. The coordinator will be responsible for completing all reports required by the funding agency. The coordinator will provide services to individuals who are at-risk of diabetes, as well as family members who participate in Diabetes Self-Management Education classes and follow-up activities. The coordinator will arrange and serve as the facilitator for the DSME classes and assist with home visits, appointments, transportation, and other support. The coordinator will assist people with diabetes to obtain services.
Document patient encounters into the electronic health record.
Complete applications for funding opportunities.
Complete reports required by funding agencies.
Complete annual Diabetes Audit.
Provide supervision of employees within the Diabetes Prevention Department.
Conduct and oversee Diabetes Prevention Program by implementing evidence-based programs.
Conduct community and individual prevention presentations and education.
Conduct and collect data for screenings, home visits, and self-management classes.
Complete quarterly Quality Assurance and Performance Improvement activities.
Coordinate and collaborate with other programs to establish diabetes prevention events for youth and adults.
Conduct annual review and update of policies and procedures.
Ensure healthcare compliance for consumers served by the diabetes prevention program.
Complete Diabetes Educator course within the first year of hire.
Other duties as assigned to maintain and enhance the operation of the department.
Must be 21 years of age or older.
High school diploma, or equivalency
Must be a graduate of an accredited university or college, a Bachelor of Science in Nursing (BSN) is preferred; however, an associate degree in nursing (ADN) may be accepted with the goal of obtaining a BSN within 2 years of employment. All nurses must be licensed as a Registered Nurse and provide evidence supporting certifications such as BLS and other necessary competencies (e.g. ACLS, PALS, etc.)
Must be a graduate of an accredited university or college, a Bachelor of Science in Nutrition/Registered Dietitian. Registered dietitians must have met the minimum academic and professional requirements to get their RD license and provide evidence supporting certifications such as BLS.
Must be a graduate of an accredited university or college, A Master of Public Health.
Must possess a valid driver license and be eligible for coverage under WRFCHC’s auto insurance policy.
Must be able to pass a criminal background check under the Indian Child Protection Act and Family Act (P.L.101-630).
First Aid/AED/CPR within 60 days of hire
Current BLS Certification or must be able to obtain it within 60 days of hire.
