Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing. Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES). Typically reports to a manager or head of a unit/department. The Diabetes Educator occasionally directed in several aspects of the work. - Provide Diabetes Education and Care to Patients and Staff in a variety of settings- Including ED, Inpatient, Critical Care, and Same Day Surgery.
- Collaborate with engaged providers and hospitalist team to deliver quality care for optimal outcomes.
- Work with committed clinical staff to assist in discharge planning and continuum of care,
- Assesses, plans, implements and evaluates learning needs of the inpatient and outpatient population
- Provides inpatient programs for nursing personnel
- Evaluates and revises as needed all resource materials for diabetes education for nursing staff.
- Utilizes research as a basis for providing and improving diabetes education.
- Participates in Care Team Rounds
- Consults with nursing staff as requested for inpatient referrals.
- Provide Diabetes Education to inpatients per assessed needs
- Provide individual and group outpatient Diabetes Education
- Participates in community diabetes education
- Plans, implements and evaluates outpatient education programs
- Participates in health screenings and/or local support group meetings.
- Participates in Diabetes Advisory Board meetings
- Provides diabetes education to women with gestational diabetes
- Assists and educates patients with the initiation and ongoing maintenance of insulin pumps
- Participates in planning, implementation and evaluation of camps/events for children with diabetes.
- Under the direction of the physician, adjusts insulin doses for optimal glycemic control
- Works collaboratively to market a comprehensive community diabetes program
- Represents WDH at community events as requested
- Provides education to primary care practices
- Provides diabetes education to off-sites as contracted.
- Participates in assuring adherence to ADA recognition standards and application process completion
- Able to manage technical equipment associated with the Diabetes Education program.
- Demonstrates skill in performing quality control tests and patients tests using a whole blood analyzer for blood sugars
- Demonstrates skill in using a variety of current blood glucose monitoring equipment and available software for downloading of results.
- Demonstrates skill in using audiovisual equipment necessary to present group education.
- Demonstrates skill in using equipment for continuous blood glucose monitoring
- Demonstrates skill in using equipment for HgbA1c monitoring
- Over three years experience in an acute care hospital; direct patient diabetes education
- Over one year experience as a Diabetes Educator; proven grant writing experience; curriculum development experience.
- Associates Degree in Nursing
- Bachelor's or Masters Degree in Nursing
- Able to present in group settings to general public and/or to other health care providers.
- Diabetes Education Certification within 3 years from date in job
- Registered Nurse License
