Activity Aide - Nursing Home jobs in Utah

Intro to the job:

Activity Living is looking for a responsible, caring leader that is passionate about working with individuals with disabilities.

As the Host Home Manager, your main areas of focus will include coordinating client care and helping them achieve their goals as well as fostering a productive partnership between clients and staff.

Responsibilities

• Be familiar with client’s goals and work together to help them make progress toward achieving them
• Set and maintain clear, appropriate boundaries with the client in your caseload
• Coordinate client care and complete client hours as assigned
• Support clients through life changes including intakes, moves, reassignments, etc.
• Assist clients with engaging in their hobbies, recreational activities, and community events
• Assist with the ongoing training and support of all Host Parents in your assigned program
• Report any field-related concerns to the Senior Manager and be prepared with proposed action plans
• Assist with crisis management and conflict resolution
• Complete compliance reviews
• Attend relevant meetings
• Complete all necessary paperwork and documentation
• Set up medical appointments for clients and confirm reliable transportation

Qualifications

• Be able to work typical hours of 10 am-5pm as well as be available for emergencies after hours
• Must be 21 years old
• Be able to pass a criminal background check
• Must maintain a valid drivers license, acceptable driving record, and current auto insurance/registration
• Reliable transportation and ability to transport clients in personal vehicle

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year

• 8 hour shift
• Monday to Friday
• On call
